H&F ‘Nothing About Us Without Us’ Disabled Residents Survey 2017

Closes 31 Jul 2017

Opened 23 Jan 2017

Overview

Earlier this year, Hammersmith & Fulham Council set up a Disabled People’s Commission to look at how to make life easier for disabled people living and working in the borough.

It is made up of local disabled people from across the borough.

We want to know about your experiences as a disabled person of local services and support.

Disabled people means all groups of people with impairments, which includes physical or hidden impairments as well as mental health.

About this survey

The survey is called ‘Nothing About Us Without Us.’  It will provide the Commission with up to date information about local disabled peoples’ lives.

This will enable disabled people to have a say in the decisions made about their support, and tackle the barriers they face to participating on an equal basis with others.

We realise there are quite a few questions and we really appreciate you giving us your time.

Please give your details at the end if you want to be entered into our prize draw to win two £20 vouchers.

The barriers which exclude people with impairments are varied and not just physical. They can include people’s attitudes and a lack of information and awareness of disabled people’s rights.

If you would like to complete this survey in another way such as on the phone, face to face or you want the questions in a different format – for example Word, Easy Read or large print - please let us know.

Please contact Kevin Caulfield (Policy Officer to Disabled People’s Commission) at Kevin.Caulfield@lbhf.gov.uk or 0778 9095049.

The commission will use this survey to put together a set of key messages into a report about your experiences to help make positive changes.

You can ask someone to help you fill in this survey or they may fill it in for you if you agree.

Keeping your personal information safe

We will keep your personal information safe and we will not identify you. Any information that might show who you are will be taken out of the results.

You do not have to give your name, but we very much hope you will share your email address with us so we can keep in touch and send you a copy of the final report.
